1. Title of Research Study.
What are the perspectives of teachers involved in the development of World Wide Web sites?

2. Project Director

3. Purpose of the Research
The purpose of this research is to understand the experiences and views of teachers whom are actively authoring World Wide Web sites.

4. Procedures for this Research
Procedures include three activities. First, there will be one observation period of about thirty minutes during which time the researcher will record the activities occurring during the regular class time of EDCI 4993, section 601 (Spring, 1999). The observation time will be evenly devideded between activities of the whole class and the intereactions of one smaller group of students. Second, one participant volunteer will be interviewed for thirty minutes outside of normal class activities. Third, one focus group of 4-6 participant volunteers will be interviewed for thirty minutes outside of normal class activities.

5. Potential Risks of Discomforts
The Project Director (listed in #2 of this form) is also the instructor for EDCI 4993, section 601 (Spring, 1999) from which participants are drawn. The risks introduced by the dual role of researcher and instructor should be considered in the light of the following: 1) participation is voluntary and will NOT be considered as part of the final grade evaluation for the class; 2) in any case, final grade evaluation for this class is based primarily on the student’s own self-evaluation; 3) the peculiar role of instructor as authority is reduced because all members of this class are educators and much of the learning will be based on peer interactions; 4) the instructor/researcher considers himself as peer to all members of this class; 5) the primary purpose for conducting this research is for the learning experience of the Project Director whom will be graded not on the quality of the research, but his reflection on his of the research process.
Participants may experience slight emotional distress, if recalling unpleasant experiences, or may become tired toward the end of the interview.

6. Potential Benefits to You or Others
Only recently have colleges of education offered educators courses of study with the goal of authoring World Wide Web sites. The results of this study can be used to improve the design of such courses.

7. Alternative Procedures
Your participation is entirely voluntary and you may withdraw consent and terminate participation at any time without consequence.

8. Protection of Confidentiality
The names of all participants and their businesses, schools and universities will be kept confidential at all times. Participants’ names, businesses, schools or universities will not be identified on any tape. The interview and focus group tapes will be transcribed by the Project Director. The signed consent forms, audiotapes, interview transcripts, and any other materials related to this project will be maintained in a secure and confidential manner by the Project Director. If the results of the study are published, participants’ names, business, schools, and universities will be disguised.

9. Signatures
I have been fully informed of the above-described procedure with its possible benefits and risks, and I have given my permission to participate in this study.
